What do you want to accomplish?
1. Review established roadmap. Review objectives and agree on agenda for session # 5. Facilitate a brief recap and review of accomplishments from last session.
2. Review action plan. Discuss progress on related aspects of the action plan and determine next steps as necessary.
3. Prepare coaches. Engage and support the working group in leading the presentation to the coaching group. Review the coaching package, including the support tools, techniques and implementation schedule. Allow appropriate time for discussion and feedback as well as opportunity for effective role-playing. Consider the individuals and their levels of experience with the coaching process and provide appropriate education and support. Working through various difficult coaching scenarios may be beneficial. Coaches should be aware of how to deal with situations of resistance and non-compliance and be assured of leadership support. If planned, participate in the Gemba.
4. Prepare information for patients and families. Consider the needs of the patients and families and prepare informational material regarding the new reporting process, including report times, patient-care benefits and specific instructions as appropriate. An example of patient information document is located in the resource section of this blog.
5. Develop an action plan. Review overall session and verify objectives achieved. Develop and agree on an action plan. Determine effective method and manner for providing communication and feedback to staff and agree on how this will be implemented and sustained. Determine and document next steps and person(s) responsible for the completion, including information rollout for staff and implementation dates and strategy.
6. Evaluate session. Encourage participants to provide feedback on effectiveness of the coaching session, noting what went well and what could have been done differently. Utilize this information to identify learning and support needs and to optimize the implementation process.
7. Prepare for follow-up and review. The working group should organize and schedule time to review and evaluate effectiveness of the new reporting process and develop countermeasure strategies as appropriate. Develop a plan for accessible and continuous evaluation and feedback process.
